A Minnesota Case Manager
“I thought no one on my caseload would be able to use the new Consumer Directed Community Supports service", a rural county case manager recently remarked. "However, I am pleasantly surprised at how well this service is working for my first client. So, now when I give people information about the service, I really feel like I am empowering them. And that feels good.”
Her first client is a 69-year old man with severe diabetes and heart disease who had been on the Elderly Waiver for several years before opting for CDCS. He and his wife prepared and implemented the community support plan in less than a week with assistance from the fiscal support entity and the county case manager. The plan allows the wife to be paid four hours each day to provide personal care and monitor his chronic health needs. With CDCS, this participant has had fewer emergency room visits for his diabetes, and his wife, who no longer works outside the home, is close by every day. His wife is much more satisfied with this care arrangement and has peace of mind knowing that her husband is getting the care that he needs.
