Arkansas' Participant Information Decal

 This static cling decal is being used for Arkansas' IndependentChoices program. The nurses or counselors use a marker and indicate the appropriate nurse and counselor for the participant in the appropriate space. The size of the decal is 4¾ X 3¼.
