Program Activities:
Michigan is offering the Self Determination in Long Term Care to all participants in the MI Choice waiver. Participants receive an individual budget developed from their plan of care and based on needs through the Person Centered Planning process. They are supported to control the use of funds comprising their budgets to hire personal assistants, contract with preferred providers, make home modifications, as well as goods and services and select from other waiver services which best meet their needs. Support Coordinators work with Participants to develop and revise budgets. Participants have the option of appointing a representative. Self Determination in Long Term Care may be utilized through the Choice Voucher System, The Agency with Choice model or a combination that includes traditional providers as well.
Self Determination in Long Term Care is moving from a "Pioneer" program to a statewide system change in fiscal year 2008.
