Program Name: Consumer-Directed Care Plus
Goals: To provide people with disabilities with more options and greater personal autonomy in determining how best to meet their long-term needs in a cost effective manner.
Target population: Consumers participating in the Consumer-Directed Care Plus (CDC+) program must be participating in the Aged and Disabled Adult (A/DA) Waiver, Developmental Disabilities (DD) Waiver or Traumatic Brain Injury/Spinal Cord Injury (TBI/SCI) Waiver at the time they seek to enroll in the CDC+ program

Program Activities:
CDC+ consumers exchange their current home and community based long-term care Medicaid services for a monthly budget. Consumers are allowed to “cash out” services on their current care and support plans and receive a monthly budget to pay for services and supports they choose. The consumers’ budgets are equivalent to the value of services they received in the Medicaid home and community-based services waiver program.
